HOSPICE CARE

M&H Elderly Care is a compassionate, caring, and offers a patient-centered approach to medical care and support for people who are at their end-of-life and their families. It is based on a philosophy of improving the quality of life when the quantity of time is limited. Our Hospice services provide emotional, physical, and spiritual support to patients and those who love and care for them.

We partner with nursing homes, hospitals, and community-based organizations to meet the specific needs of terminally ill individuals and their families. M&H Elderly Care provides most of this Hospice care in patients’ homes. However, care can also be provided in senior living residences and nursing homes which may include assisted and independent living. In cases where a patient may need care that is beyond what we can provide at home, we have relationships with local hospitals and other facilities to take care of this, and we will also provide support for a seamless transition back to their home.

Hospice Services for End of Life Support

At M&H Elderly Care we support our patients and families as they approach the end of their life. We will help them understand the available options open to them and work to support them in making their decisions. We provide customized care to coordinate the services that will make them comfortable and safe.

Care Services

Our hospice care team

Our hospice team of specialist meets regularly to analyze and review your care plan to serve you with the most appropriate options available.

To help with this, our hospice care includes the following:

    • Pain management and Full-spectrum patient comfort care and pain management
    • Support and Guidance for families, including education on the bereavement support, disease process, and improving the quality of life for the entire family. By providing personalized social, spiritual, and medical care. Our hospice team gives patients the opportunity to create cherished memories with their families and also enjoy life to the fullest.
    • Attending Physician:

      Prescription and oversees medical care to manage symptoms and pain

    • Hospice Medical Directors:

      Liaise between the family, patient, and attending physician to get insights into the patient’s needs and fashion a way of solving those needs.

    • Social Worker:

      Advising the families and patients when needed, develop an integrated care plan based on physical, emotional, financial, social needs, and also assists with bereavement support when a loved one has passes on

    • Spiritual Counselor:

      Draws on the values and beliefs of families and patients to cope with the spiritual aspects of loss and illness.

    • Volunteers:

      We offer support for all non-medical needs such as running errands, assisting with family needs, providing socialization and companionship.

Rehabilitation Care is a service provisioned for patient in the process of healing from injury or surgery of any kind. Normally family loved ones offer their services during the process. In a times when a patient or client lives far from their loved ones, they may need help to be able to move around or have some errand complete, and this is where our service comes to play. We provide them with a well trained caregiver to help them in transitioning to get their task easier during such period.
